Registration/ Membership
A non-refundable annual Membership fee of $30.00 per student or $50.00 per family is required at the time of registration and each September 1st thereafter. This Membership fee will be due upon registration and due annually each year on September 1st thereafter. If a student drops out for more than 30 days (1 month), a membership fee will be due upon re-enrollment. If a student enrolls 60 days prior to September 1st (June 1st), the annual membership fee will carry over to the following year.
Assumption of Risk
RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK; AND INDEMNITY AGREEMENT (“AGREEMENT”)
I represent that I understand the nature of this activity and that I am qualified, in good health and in proper physical condition to participate in such activity. I acknowledge that if I believe event conditions are unsafe or I am unable to safely perform any activity, I will immediately discontinue participation in the activity.
I fully acknowledge, understand, appreciate and agree, that this activity involves risks of serious bodily injury, including permanent disability, paralysis and death, which may be caused by my own actions, or inactions, those of others participating in the event, the conditions in which the event takes place, or the negligence of the Releasees named below; and that there may be other risks either not known to me or not readily foreseeable at this time; and I fully accept and assume all such risks and all responsibility for losses, cost, and damages I incur as a result of my participation in the activity.
I further acknowledge, understand, appreciate and agree that my participation may result in possible exposure to and illness from infectious diseases, including, but not limited to, MRSA, Influenza, and COVID-19. While particular rules and personal discipline may reduce this risk, the risk of serious illness and death does exist. I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releasees or others, and assume full responsibility for my participation and exposure.
I hereby release, discharge, and covenant not to sue your business, it's administrators, directors, agents, officers, volunteers, employees, contractors, other participants, any sponsors, advertisers, and, if applicable, owners and lessors of the premises on which the activity takes place, (each considered one of the “RELEASEES” herein) from all liability, claims, demands, losses, damages, on my account caused or alleged to be caused in whole or in party by the negligence of the RELEASEES or otherwise, including negligent rescue operations and further agree that if, despite this release, waiver of liability, and assumption of risk, I or anyone on my behalf, makes a claim against any of the RELEASEES, I will indemnify, defend, and hold harmless each of the RELEASEES from any loss, liability, damage, or cost, which any may incur as the result of such a claim.
I have read the RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK AND INDEMNITY AGREEMENT, and I understand that I have given up substantial rights by signing it and have signed it freely and without any inducement or assurance of any nature and intend it to be a complete and unconditional release of all liability to the greatest extent allowed by law. I agree that if any portion of this Agreement is held to be invalid, the balance, notwithstanding, shall continue in full force and effect.
CONSENT I, hereby covenant and promise that I am the minor’s parent and/or legal guardian, and on behalf of myself and the minor, understand the nature of the above referenced activities and the minor’s experience and capabilities and believe the minor to be qualified to participate in such activity. I further understand the risk of exposure to injury and/or infectious diseases, for myself and my child, as a participant, spectator at events, classes or our presence at the facility. I hereby release, discharge, covenant not to sue and AGREE TO DEFEND, INDEMNIFY AND HOLD HARMLESS each of the RELEASEES from all liability, claims, demands, losses or damages on the minor’s or my account caused or alleged to have been caused in whole or in part by the negligence of the Releasees or otherwise, including, but not limited to injury, negligent rescue operations, and/or exposure to infectious diseases and I further agree that if, despite this release, I, the minor, or anyone on the minor’s behalf makes a claim against any of the above RELEASEES, I WILL DEFEND, INDEMNIFY, AND HOLD HARMLESS each of the RELEASEES from any litigation expenses, attorney fees, loss liability, damage, or cost which any RELEASEE may incur as the result of any such claim.
As the legal parent/guardian or adult student, I release and hold harmless Meraki Dance Expressions, its owners and operators from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Meraki Dance Expressions, its owners and operators or in route to or from any of said premises.
Release of Liability
RELEASE OF LIABILITY, PHOTOS & VIDEOS: I release and hold harmless Meraki Dance Expressions, its owners and operators from any and all liability, claims, demands and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Meraki Dance Expressions, its owners and operators or in route to or from any said premises. I further release the use of any photos and/or videos of myself or my child to Meraki Dance Expressions while participating in any enrolled classes.
Release of Photo/Video Images
I hereby agree on my own behalf if I am 18 years of age or older, or on behalf of the minor/minors named on this family account grant permission to Meraki Dance Expressions and its agents or employees, to use photographs or videos taken of me or the minor/minors named on this family account for use in studio publications including but not limited to brochures, newsletters, and magazines, and to use the photographs or videos on display boards, and to use such photographs or videos in electronic versions of the same publications or on websites or other electronic forms or media, and to offer them for use or distribution in other non-studio publications, electronic or otherwise, without notification to me or any other person.
I hereby waive any right to inspect or approve the finished photographs or videos or printed or electronic matter that may be used in conjunction with them now or in the future, whether that use is known to me or unknown, and I waive any right to royalties or other compensation arising from or related to the use of the photographs or videos, and if signing on behalf of the minor/minors named on this family account, I waive all such rights on behalf of minor/minors named on this family account. I am 18 years of age or older and I am competent to contract in my own name or I am the parent or legal guardian of the minor/minors on this family account. I have read this release before signing below, and I fully understand the contents, meaning and impact of this release.
Medical Emergencies
The undersigned gives permission to Meraki Dance Expressions, its owners and operators to seek medical treatment for the participant in the event they are not able to reach a parent or guardian. I hereby declare any physical/mental problems, restrictions, or condition and/or declare the participant to be in good physical and mental health. Payment Policy
Fees are based on an annual student rate. Special Discount Rates are applicable for immediate family members only within the same household. Rates are based on the total number of monthly student classes. All tuition payments will be due monthly on the 1st of each month. Tuition is late if not paid by the 10th of the month. A late fee of $15.00 will be added on all late accounts. No refunds, pro-rates, or credits will be given for missed classes.
Method of Payment
Our required method of payment is to set up an automatic electronic payment for all tuition. Tuition can be paid by automatic payment through your credit card by logging in to your student portal and paying by credit card monthly or in person at the front desk by check or cash. All checks or ACH accounts returned for non-sufficient funds will be charged a $30.00 fee.
Adding, Withdrawing and Refunds
Enrollment and Tuition Payments are active year round until the studio administration is given a 14 day written notice to drop a class(s). Once this 14-day written notice is received, the student's account will be charged on the day the cancellation notice is received for the full amount due 30 days out. Any and all fees that are currently due will be processed the day the cancellation notice is received.
If any class would like to be dropped, notice must be given in writing, by email, letter or completing a Drop Notice at the front desk. If the studio is not notified in writing, the regular monthly tuition as well as late fees will continue to be added to your account until notice is given. A minimum of 14 days cancellation notice is required to drop a class. Please note all balances must be paid prior to withdrawal and then all automatic payments will be terminated. Meraki Dance Expressions reserves the right to terminate lessons for any student without notice. In such a case a refund for unused lessons will be given.
Please note: enrollment in classes is NOT automatically canceled after our annual recital in June. MDE automatically enrolls students in the appropriate classes after each season if finished and unless told otherwise all tuition and membership fees will be due and processed. Payment for adding a class must be processed before starting a class.
Dress Code & Attendance and Lateness
Dress Code
Required dance-wear and dance shoes should be worn to all classes. Failure to wear required dance-wear to class can result in students being asked to leave or sit out the class. Hair for all classes must be securely pulled back away from the face. The Studio Dress Code is available on the website under "Dress Code".
Attendance and Lateness
Regular attendance is required to have a successful learning experience. We ask that illness be the only reason for missed classes. Make-up classes are at the discretion of the instructor. Make-ups are limited to 6 per student, per class, per year and must be made up within 30 days of the missed class. Students should arrive on time to class. Students who miss the first 15 minutes of class may be asked to sit out the class for their own safety.
